I, the undersigned, acknowledge and agree to the following terms and conditions in consideration of being allowed to participate in axe throwing at Small Axe:

1. Assumption of Risk: I understand that axe throwing includes inherent risks, including but not limited to the risk of personal injury, property damage, and even death. I assume full responsibility for my participation in this activity and accept all risks associated with this activity.

2. Intoxication Policy: I acknowledge that Small Axe reserves the right to refuse service to patrons who appear intoxicated. If deemed intoxicated, I understand that I may be asked to leave the premises.

3. Eyewear and Attire: I am aware that optional eyewear will be provided but is not mandatory. I accept that if I choose not to wear protective eyewear that results in injury, I release Small Axe from all liability associated with the injury. I agree to wear closed-toed shoes during my participation in the axe throwing activity.

4. Age Requirements: I confirm that I am at least 18 years old and will provide valid identification to verify my age before participating. Patrons aged 16 to 17 may participate if accompanied by a guardian.

5. Health Condition: I agree to remove myself from participation if I sense or observe any unusual, hazardous, or unsafe conditions or if I feel that I have experienced any deterioration in my physical, mental, or emotional fitness.

6. Waiver of Liability: In consideration of my participation in this activity, I hereby waive, release, and discharge Small Axe, its owners, employees, instructors, and agents from any and all claims, demands, or causes of action arising from my participation in the axe throwing activity, even if the injury or damage is caused by negligence.

7. Agreement Not to Sue: I agree that I will not sue or make any claim against Small Axe for any loss, injury, costs, or damages of any kind relating to my participation in the axe throwing activity.

8. Indemnification: I agree to indemnify and hold Small Axe harmless from any claims made by me or on my behalf as a result of my participation in this activity.

9. Media Release: I acknowledge that Small Axe has the right to take photos and videos of patrons within the premises and use them for promotional purposes on social media and other marketing platforms.

10. Understanding of Terms: I acknowledge that I have read this waiver and fully understand its terms. I am signing this document freely and voluntarily, with the knowledge that I am waiving certain rights.
